A valid insurance card/ documentation, photo ID and credit or debit card number is required for persons presenting with insurance.
Eligibility will be verified with your insurance carrier electronically at the time of your visit.
The credit card number will be used to secure your account towards future balances that may become due once your insurance carrier has decided what amount is your responsibility.
